Here are the forms that must be completed before an athlete can participate in sports at Berlin High School.
1. The copy of the training rules and Permission Slip forms must be completed and returned to your coach before each season that you wish to compete in athletics at BHS. The form from the fall season does not carry over to the winter etc.
The training rules form must be signed by both parent and athlete.
2. A current physical is required to participate in any athletic department activities. The physical form must be returned to the Nurses Office before an athlete may practice or participate in any way. Only the Athletic Department Physical form is acceptable - the CT State Health Assessment is NOT a substitute. A current physical is one that was performed within a year of the first date of practice. It is possible that a physical, that was current for the Fall season, may be out-of-date by the spring season. Please check with the Nurse to make sure that your physical is current - a list is maintained in the Health Room and all coaches are sent a copy to check physical dates. Coaches will not allow you to participate if your physical is out-of-date.

Note: These forms are referred to as the Yellow Forms in the student handbook. If you are printing them from Home, they do not have to be printed on Yellow Paper - they are still valid if they are printed on white paper as long as a physician signs the form.
